• Cybex International Treadmills Recalled Due to Fall Hazard
 
Cybex International Inc., of Medway, Mass., in cooperation with the CPSC, initiated a recall on October 29, 2008 of about 19,000 Cybex treadmills, as the treadmills can speed up unexpectedly while in use due to a malfunction with the lower control board, thus posing a fall hazard to consumers. Cybex International has received 24 reports of incidents involving the treadmill unexpectedly increasing speed, including six reports of consumers falling. Three of those incidents resulted in minor injuries. The recall involves the (Cybex 445T, 455T, 530T, 450T, 500T, 515T, and 520T treadmill models. “Cybex” and the model number are written on the console. The recalled treadmills were sold by Cybex International and Cybex dealers nationwide from January 2001 through September 2008. Consumers should immediately unplug and stop using the recalled treadmills and contact Cybex to receive a free replacement fuse. If requested, a Cybex technician can install the fuse free of charge. • Trade Associates Group and the Crate and Barrel Recall Ghost Tealight Holders Due to Fire and Burn Hazard
 
Trade Associates Group Ltd., of Chicago, Ill. and Crate & Barrel, of Northbrook, Ill., in cooperation with the CPSC, initiated a recall on October 29, 2008 of about 30,000 Ghost tealight holders, as they can ignite, posing a fire and burn hazard to consumers. There have been five reports of the tealight holders igniting and one report of the holder becoming excessively hot. Two consumers suffered burns to the hand. In addition, minor property damage to furniture was reported. The recalled product is a metal tealight candle holder in the shape of ghost. The product is white with a black base. They were sold by Crate & Barrel as a set of 3 (SKU #370-846 and/or Catalog Item #X2006) and by other retailers as individual units (SKU #770123). The SKU can be located on the product’s packaging. The recalled product was made in China and sold by Crate & Barrel’s October catalog and stores, and from houseware retail stores nationwide from August 2008 to October 2008. Consumers should stop using the Ghost Tealight Holders immediately and return them to the store where purchased for a full refund. • Coyne & Company Halloween Figurines Recalled Due to Lead Paint Hazard
 
Coyne’s & Company Inc., of Minneapolis, Minn., in cooperation with the CPSC, initiated a recall on October 29, 2008, of about 14,000 Casper the Friendly Ghost® Halloween Mini-Figurines, as the recalled figurines contain high levels of lead in the paint. No injuries have been reported. The recalled mini-figurines are made of resin and paper, and are three inches tall. They were sold in mini bags decorated with a spider and web design. Casper the Friendly Ghost® figurines are dressed as: Dracula in a red spider bag, Skeleton in a green spider bag, Devil in a purple spider bag, Mummy in an orange spider bag, Casper® in a blue spider bag, and Wendy the Witch® in a yellow spider bag. The figurines are stamped with TM & ©Harvey Ent and “Coyne’s & Company” and “CP1030” are printed on a white sticker on the bottom of the bags. The recalled figurines were made in China and sold by gift, specialty, card, and drug stores nationwide from June 2005 through September 2008. Consumers should immediately take the recalled figurines away from children and contact Coyne’s & Company to receive a full refund or credit. • Earth Friendly Wooden Toys Recalled Due to Choking Hazard
 
Earth Friendly LLC, of Beaverton, Ore., in cooperation with the CPSC, initiated a recall on October 30, 2008, of about 1,000 wooden toys, as the small parts can detach and break, posing a choking hazard to young children. In addition, the size of the rattle handle violates voluntary rattle standards. No injuries have been reported. This recall involves three models of wooden toys. Moee the car, Cubby the stackable bear and the Bell rattle. The toys are painted in glossy red, orange, green, red, black and yellow. The recalled toys were sold by toy specialty stores in Alaska, California, Colorado, Montana, Nevada, Oregon, and Washington from April 2008 through September 2008. Consumers should immediately take the recalled toys away from children and contact Earth Friendly to exchange or refund the product. • Homelite Consumer Products Announces Recall to Repair Chain Saws Due to Chain Brake Failure
 
Homelite Consumer Products Inc., of Anderson, S.C., in cooperation with the CPSC, initiated a recall on October 30, 2008, of about 370,000 Homelite chain saws, as the chain brake can fail to stop the chain on its first application, posing a risk of laceration to consumers. No injuries have been reported. This recall involves Homelite brand chain saws with the following model numbers: UT10514, UT10516, UT10517, UT10518, UT10520, UT10540, UT10560 and UT10918. Affected chain saws have a manufacture date between November 2007 and August 2008. Both the model number and the date of manufacture appear on the chain saw’s data plate, located beneath the rear handle of the saw. Products with a green “dot” sticker under the bottom side of the handle area and on the outside of the package are not subject to this recall. The recalled chainsaws were made in China and sold exclusively by Home Depot stores nationwide between December 2007 and October 2008. Consumers should immediately stop using the chain saws and contact Homelite Consumer Products to locate the nearest authorized service center to schedule a free repair. • PC Notebook Computer Batteries Recalled Due to Fire and Burn Hazard
 
Hewlett-Packard, Toshiba and Dell Notebook Computers, in cooperation with the CPSC, initiated a recall on October 30, 2008, of about 35,000 batteries (an additional 65,000 batteries were sold worldwide), as these lithium-ion batteries can overheat, posing a fire and burn hazard to consumers. There have been 19 reports of the batteries overheating, including 17 reports of flames/fire (10 resulting in minor property damage). Two consumers experienced minor burns. The recalled batteries were included with, and sold separately for use in the following notebook computer models:

The battery model is located on the battery label. They were made by Sony Energy Devices Corporation, of Japan and computers with the recalled batteries were sold directly by Hewlett-Packard, Toshiba and Dell, through computer and electronics stores nationwide, and through various Web retailers. Hewlett-Packard sold the recalled batteries from December 2004 through June 2006; Toshiba sold them from April 2005 to October 2005, and Dell shipped them between November 2004 and November 2005. Consumers should immediately remove the recalled battery from their notebook computer, and contact their computer manufacturer to determine if their battery is included in the recall. If it is, they may request a free replacement battery. After removing the recalled battery from their notebook computer, consumers may use the AC adapter to power their computer until a replacement battery arrives. Consumers should only use batteries obtained from their computer manufacturer or an authorized reseller. • MEDport LLC Fit & Fresh ™ Mixers Recalled Due to Laceration Hazard
 
MEDport LLC, of Providence, R.I., in cooperation with the CPSC, initiated a recall on October 30, 2008, of about 54,500 Fit & Fresh™ Smooth Blend Mixers, as placing the cup on or off the base of the mixer can unexpectedly turn it on, thus activating the blade. This can pose a serious laceration hazard to consumers. MEDport has received one report of finger lacerations that required medical treatment. This recall involves Fit & Fresh™ Smooth Blend Mixers with item number 770FF and model number SB-19. The white mixers have four components – a base unit, blade assembly, blending cup and cap. Fit & Fresh™ is printed on the front of the base. The recalled mixers can be identified by the date code, a four-digit number located on the bottom of the base. Date codes included in the recall are: 2707, 4407, 5207, 0308 and 0508. The recalled mixers were made in China and sold by Longs, GNC and Vitamin Shoppe stores nationwide and on http://www.amazon.com from September 2007 through August 2008. Consumers should immediately stop using the mixers and determine if their product is subject to this recall. • DYMO Power Adaptors sold with DYMO® Printers Recalled Due to Burn Hazard
 
DYMO, of Stamford, Conn., in cooperation with the CPSC, initiated a recall on October 30, 2008, of about 17,000, DYMO® LabelWriter® Power Adapters used with printers, as the power adapters can fail, causing the printer to overheat. This can pose a burn hazard to consumers. DYMO has received 11 reports of printers overheating and emitting smoke and/or a burnt smell. No injuries have been reported. This recall involves the external power adapters sold with the following DYMO LabelWriter® 400 series printers: DYMO LabelWriter 400, DYMO LabelWriter 400 Turbo, DYMO LabelWriter Twin Turbo, DYMO LabelWriter Duo and DYMO Desktop Mailing Solution with housing date codes 2407, 2507, 2607 or 2707. The date code is printed on the plastic housing of the adapter. Power adapters with “RoHS” printed on the top right of the product label are not included in this recall. The recalled adaptors were made in China and sold by office supply stores, discount retailers, and various specialty retailers nationwide from September 2007 through October 2008. Consumers should immediately unplug the recalled power adapter from the wall socket and from their printer and contact the firm to receive a free replacement external power adapter kit. • Rage Wireless Guitars Used with Popular Video Gaming System Recalled Due to Chemical Burn Risk
 
Performance Designed Products LLC, of Sherman Oaks, Calif., in cooperation with the CPSC, initiated a recall on October 22, 2008 of about 57,000 Rage wireless guitars, as the A circuit board defect can cause AA batteries used in the guitar to leak if they are installed incorrectly, posing a risk of chemical burns to consumers. Performance Designed Products has received one report of a burn. The Rage Wireless Guitar controller is designed to be used on the Nintendo Wii system. The guitar is blue or white and is 31 inches long. The guitar contains battery-operated LED-lighted fret buttons that go up the neck of the guitar. The recalled guitar was made in China and sold by mass merchandisers and specialty retailers nationwide from June 2008 through September 2008. Consumers should stop using the wireless guitar game controllers immediately and return them to the place where purchased for a full refund. Do not contact or return the product to Nintendo. • Fiesta Recalls to Inspect and Repair Gas Grills Due to Fire and Burn Hazards
 
Fiesta Gas Grills, of Dickson, Tenn., in cooperation with the CPSC, initiated a recall on October 22, 2008 of about 47,000 Blue Ember Gas Grills, as they can be assembled improperly exposing the gas burner hoses to excessive heat, thus posing fire and burn hazard to consumers. Fiesta has received 14 reports of grill fires. No injuries have been reported. The recall involves Blue Ember liquid propane (LP) or natural gas outdoor grills; recalled model and serial numbers are listed below. The model and serial numbers are printed on a rating plate label on the rear of the grill. The cabinet style grill has two doors and is silver and black or silver and gray. “Blue Ember” is printed on the grill’s hood.

The recalled grills were made in China and sold at various home centers and retailers nationwide from November 2007 through June 2008. Consumers should immediately stop using the grills, and inspect the burner hoses to make sure they have been properly assembled. If the hose is to the rear of the installed heat shield, the grill has been improperly assembled and consumers should contact Fiesta for replacement hoses, assembly instructions, and if necessary, for assistance in examining the grill. • YMCA Trading Lighters Recalled Due to Potential Burn Hazard to Children
 
YMCA Trading Inc., of Maspeth, N.Y., in cooperation with the CPSC, initiated a recall on October 23, 2008, of about 24,000 “Gas Match” and “Ready, Aim…Fire” Multi-Purpose Lighters, as the lighters fail to meet federal safety standards because they might not contain a child-resistant mechanism, posing a potential fire and burn hazard to young children. No injuries have been reported. The recalled “Gas Match” lighters have either a multi-colored red/white, blue/white or green/white plastic body, and a silver metal nozzle. Each lighter measures 10½-inches long. The recalled “Ready, Aim…Fire” lighters have either a solid black, blue, red or green plastic body, and a silver metal nozzle. Each lighter measures 9-inches long. The recalled lighters were made in China and sold by retail stores in New York State from August 2008 through October 2008 for about $1. Consumers should immediately stop using the recalled lighters and contact YMCA Trading to arrange for return of the lighter for a refund. • Shimano American Corp. Bicycle Wheels Recalled as a Flat Tire Poses Risk of Injury to Rider
 
Shimano American Corp., of Irving, Calif., in cooperation with the CPSC, initiated a recall on October 23, 2008, of about 275 Shimano Dura Ace Carbon Clincher Wheels for Bicycles, as the rim surface and spoke hole plugs can cause a puncture to the inner tube, resulting in a flat tire. This can cause the rider to lose control and fall. No injuries have been reported. This recall involves the Shimano Dura Ace Carbon Clincher Wheel Sets with model number WH-7850 C24CL sold for road racing bicycles. The rim has labels that read “Shimano WH-7850,” “Dura Ace,” and “Carbon 1380.” Wheels are intended for use with high end adult road racing bicycles. The recalled bicycles were sold by bicycle specialty stores and dealers nationwide from April 2008 through August 2008. Consumers should stop using the bicycles with the recalled wheels, remove the wheels, and return them to their local bike dealer or retailer for a free inspection and repair. • LeapFrog Gaming System Batteries and Recharging Station Recalled Due to Burn Hazard
 
LeapFrog Enterprises Inc., of Emeryville, Calif., in cooperation with the CPSC, initiated a recall on October 23 2008, of about 35,500 rechargeable batteries and recharging stations for Didj Custom Gaming Systems, as the rechargeable batteries included with the Recharging Station can overheat if the gaming system is placed into the recharging base upside down, posing a burn hazard to consumers. LeapFrog has received 12 reports of the rechargeable batteries overheating, including one report of a minor hand burn. The Recharging Station comes with two rechargeable batteries, an AC adapter, and a recharging base. The recharging base is white with a green indicator light on the bottom. The Recharging Station for Didj was sold as item number “30676”, which is printed on the box and on the bottom of the recharging base. The recalled product was made in China and sold by department stores and toy stores nationwide, www.leapfrog.com, and other online retailers from July 2008 through October 2008. Consumers should immediately stop using the recharging base and the two rechargeable batteries and contact LeapFrog for a full refund. Consumers can continue using the AC adapter connected directly to the Didj handheld. • Battery-Biz Notebook Computer Power Adapters Recalled Due to Burn Hazard
 
Battery-Biz Inc., of Camarillo, Calif., in cooperation with the CPSC, initiated a recall on October 28, 2008, of about 1300 Duracell 130W Combo Power Adapters used with notebook computers, as the adapters can fail and overheat. This can pose a burn hazard to consumers. Battery-Biz has received five reports of power adapters overheating. No injuries have been reported. This recall involves the Duracell 130W Combo AC/DC adapters with model numbers EA10900, AC-6501, and DRUM130 and date codes 0804 and 0805. The black power adapter is used with notebook computers. “DURACELL UNIVERSAL POWER ADAPTER” is printed on the front of the power adapter. The model and date codes can be found on the rating label on the back of the power adapter. The recalled products were made in China and sold online at www.dell.com and www.duracelldirect.com from February 2008 through March 2008. Consumers should immediately stop using the power adapter and contact Battery-Biz to return it and obtain a free replacement. • Serta International Mattresses Recalled Due to Violation of Federal Mattress Flammability Standard
 
Serta International Mattress Co., of Hoffman Estates, Ill., in cooperation with the CPSC, initiated a recall on October 28, 2008, of about 5,100 Serta Zipper-Covered Foam Core Mattresses, as they fail to meet the mandatory federal open flame standard for mattresses, posing a fire hazard to consumers. No injuries have been reported. The recalled mattresses have a zipper cover over a foam core. They were sold in twin, full, queen, king and California king sizes and they were manufactured between November 30, 2007 and May 20, 2008. The mattress name, date of manufacture, model, and serial numbers are located on label attached to the mattress. Only specific serial numbers are included in the recall. The recalled mattresses are listed on the chart below.

The recalled mattresses were sold by mattress retailers nationwide from December 2007 through May 2008. Consumers should immediately contact Serta to determine if their mattress is included in the recall, and if it is, to receive a free replacement mattress cover. Consumers can install the cover themselves or request free installation by a qualified repair technician. • Infant Death Prompts Recall To Repair 600,000 Drop Side Cribs By Delta Enterprise; Spring Peg Failure Can Cause Entrapment and Suffocation
 
Delta Enterprise Corp., of New York, N.Y., in cooperation with the CPSC, initiated a recall on October 21, 2008, of about 600,000 drop side cribs, as the crib’s drop side can detach when the spring peg is not engaged, which can cause an entrapment and suffocation risk to infants and toddlers. If the crib’s spring pegs in the base of each leg are pushed back into the lower track, the pegs can become nonfunctional. This can cause the drop side to detach from the crib and create a gap that can entrap an infant or toddler and cause suffocation. One incident resulted in the death of an 8-month-old child. There was one report of entrapment of a child and one incident in which the drop side detached due to a disengaged spring peg.

The recall involves all Delta drop side cribs manufactured prior to 2006 that use “Crib Trigger Lock with Spring Peg” drop side hardware design. The recalled model numbers and country of origin include: 4340, 4343, 4520, 4600, 4620, 4624, 4625, 4629, 4660, 4665, 4720, 4750, 4751, 4850, 4855, 4857, 4880, 4920, 4925-2, 4925-2B, 4925-6, 4980, and 8605. The cribs were made in China and were sold at major retailers nationwide from January 2000 through January 2007.

CPSC and the CPB would like to remind parents not to use any crib with missing, broken, or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop side crib parents should check to make sure any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap and entrap a child. In addition, do not try to repair any side of the crib, especially with tape, wire or rope. Parents and caregivers are encouraged to find a safe, alternative sleep environment for their child until the repair kit, with new safety pegs, are safely installed on the recalled cribs.

• Infant Death Prompts Recall To Repair 985,000 Delta Enterprise Drop Side Cribs; Missing Safety Pegs Can Cause Entrapment and Suffocation Hazards
 
Delta Enterprise Corp., of New York, N.Y., in cooperation with the CPSC, initiated a recall on October 21, 2008, to replace missing safety pegs involving 985,000 drop side cribs. Failure to use or install safety pegs can cause an entrapment and suffocation risk to infants and toddlers. When the safety pegs in the base of each leg of the crib are missing from the lower track, the crib locks can disengage and detach if lowered below the peg hole, creating a hazardous gap. This gap can lead to the entrapment and suffocation of an infant or toddler. The death of an 8-month-old child has been reported. CPSC is also aware of two other entrapments and nine other disengagement incidents in cribs where the safety pegs were missing.

The recall involves all Delta cribs manufactured in Taiwan or Indonesia, with the “Crib Trigger Lock with Safety Peg” drop side hardware design. These model numbers and country of origin can be located on the mattress support board label: 4320, 4340, 4500, 4520, 4530, 4532, 4540, 4542, 4550, 4551, 4580, 4600, 4620, 4624 (production dates 01/06 thru 11/07), 4640, 4660, 4720, 4735, 4742, 4750 (production dates 01/95 thru 12/00), 4760, 4770, 4780, 4790, 4820, 4840, 4850, 4860, 4880, 4890, 4892, 4900, 4910, 4920, 4925-2, 4925-6, 4930, 4940, 4943, 4944, 4947, 4948, 4949, 4950, 4958, 4963, 4968, 4969, 4980. The cribs were sold at major retailers, including Walmart, Kmart and Target.com from January 1995 through September 2007. These cribs have also been found to be sold secondhand.

CPSC would like to remind parents not to use any crib with missing, broken, or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop side crib parents should check to make sure all moving parts operate smoothly. Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap and entrap a child. In addition, do not try to repair any side of the crib, especially with tape, wire or rope.

GN Netcom Inc., of Nashua, New Hampshire, in cooperation with the CPSC, initiated a recall on October 14, 2008 of about 525,000 (an additional 675,000 were sold outside the US) GN9120 Wireless Headsets with ATL lithium-ion polymer batteries, as internal short circuit can cause the batteries to overheat, posing a fire hazard. GN Netcom has received 10 reports of incidents involving overheating, including three reports of open flames and property damage to furniture on which the headsets were resting. An additional 37 reports of overheating, 3 reports of open flames and 1 report of second degree burns, requiring medical attention, was received outside the U.S. The headsets are intended primarily for professional use in offices and call centers. "GN Netcom" or "GN9120" is printed on the base station and headset. The affected batteries have part number 603028 and have a white plastic enclosure. The batteries are labeled "Made by ATL (Amperex Technology Ltd.)" and "(ATL P/N 603028)." Batteries sold as a replacement part are labeled "GN9120 battery replacement kit." The recalled headsets were made in China and sold by GN Netcom, authorized distributors and resellers nationwide from January 2005 through September 2008. Dollar General Merchandising Inc., of Goodlettsville, Tenn., in cooperation with the CPSC, initiated a recall on October 15, 2008, of about 200,000 Battery Operated Toy “Speed Boats,” as the two battery terminals can come into contact with each other, causing it to overheat, thus posing a burn hazard to consumers. Dollar General has received two reports of the batteries overheating however, no injuries have been reported. The recalled boats are lightweight plastic toy boats supported by an inflatable hull with “outboard” motors on them. The motor uses two AA batteries. The toy boat measures about 12 inches long by 8 inches wide and comes in various colors and designs. They were made in China and sold by Dollar General nationwide from March 2008 through July 2008. • Playkids USA Convertible Cribs Recalled Due to Entrapment and Suffocation Hazards
 
Playkids U.S.A. of Brooklyn, N.Y., in cooperation with the CPSC, initiated a recall on October 16, 2008, of about 2,000 convertible cribs, as the sides of the convertible crib are made of a mesh that expands, creating a gap between the mattress and the side through which an infant can slip. This poses suffocation and entrapment hazards for young children. CPSC is aware of the death of a 5-month-old child on August 31, 2008 in Brooklyn, N.Y. The child became entrapped between the mattress and the stationary side rail of the convertible crib and suffocated. This recall involves the Playkids U.S.A. convertible crib/playpen/bassinet/bed with model number PLK-909. The convertible cribs have a drop side rail, a stationary side rail, a canopy assembly, and a bassinet. The convertible cribs, which were made in China, were sold in juvenile product retailers in New York from March 2007 through September 2008. • BRP U.S. Inc. Youth Terrain Vehicles Recalled Due to Collision Hazard

Pottery Barn has received 12 reports of injuries requiring medical attention including lacerations, neck and back pain, bruising, and one incident involving fractured ribs and about 50 reports of the hammock stand breaking without injury. The wooden hammock stand measures about 154 ½ inches long by 53 ½ inches in height by 53 inches in width. There are two black metal brackets on the base and on the arms of the hammock stand. All Pottery Barn two pole wooden hammock stands sold from 2003 through 2008 are included in this recall. No injuries were reported. The recall involves Effikal RVGP-PC Gas Vent Damper size 4, 5, 6, 7, 8, 9, or 10 which are installed with the gas boiler systems listed below. The vent damper date code is located on a sticker under the motor assembly cap on the plastic base. Serial numbers are located on a label on the side of the damper’s cap. Hasbro and CPSC have received 46 reports of injury to children ages 4 to 12, including reports of bruising, abrasions, pinch marks, blood blisters and welts. The toy blaster is yellow with a black handle and an orange plunger, trigger, and reload clip. The word “NERF” is in black lettering on both sides of the blaster and the word “ARMED” is written on the orange plunger. “RECON CS-6” is posted on the gray cocking mechanism. There are five interchangeable parts including a shoulder stock, a flip-up sight, a barrel extension, a quick re-load clip and a dual-mode light beam. The recalled wood abacus is a multi-sided activity center that has a chalkboard, movable block letters with pictures, sliding shapes, a fun maze with gears, and a blue and yellow clock on the side of the activity center with two pink moving hands. No injuries have been reported. Only the Gotham compact fluorescent ceiling lights with the optional backup battery pack are included in the recall. Jo-Ann Stores Inc., of Hudson, Ohio , in cooperation with the CPSC, initiated a recall on September 24, 2008, of about 6,000 SmartSpace™ Papasan chairs, as the chairs were sold without a warning label instructing consumers to lock the chair’s legs before sitting. This poses a fall hazard to consumers. Jo-Ann has received five reports of chairs collapsing when the legs were not locked however, no injuries have been reported. The chair has a circular fabric seat that sits on a 10-inch-tall base. The fabric seats were sold in purple, pacific, lime yellow and gray. The recalled chairs were made in China and sold by Jo-Ann Fabric and Craft stores nationwide from June 2008 through September 2008. Consumers should immediately stop using the chair until a warning label, instructing consumers to engage the legs in the locked position before use, has been obtained from either Jo-Ann Fabric or other Craft Stores. Igloo Marine Elite Coolers, in cooperation with the CPSC, initiated a recall on September 24, 2008, of about 7,200 Igloo Marine Elite Coolers, as the sharp edges on the stainless steel latch attached to the cooler can pose a laceration hazard to consumers. Igloo Products has received eight reports of lacerations requiring stitches on the consumer’s legs or arms. This recall involves Marine Elite coolers sold in white with stainless steel latches. The coolers were sold in four sizes: 72 quart, 94 quart, 128 quart and 162 quart. The recalled coolers were sold by West Marine stores nationwide from January 2008 to July 2008. Studio RTA, of Pico Rivera, Calif., in cooperation with the CPSC, initiated a recall on September 24, 2008 of about 48,600 TV stands, as the stability of the stands does not meet industry standards to prevent a television from tipping over, thus posing a risk of injury or death to consumers. This recall involves four different TV stands “E Series” Model 060331: The product consists of two glass shelves, a lower shelf made of metal and black leg and frame components. Razor USA LLC, of Cerritos, Calif., in cooperation with the CPSC, initiated a recall on September 25, 2008 of about 103,000 Razor® PowerWing™ Three-Wheeled Scooters, as the undersides of the foot platforms can have sharp edges, posing a laceration hazard to children. Razor has received 10 reports of Achilles tendon or ankle lacerations, including four consumers who required tendon surgery and three who needed stitches. TCB Imports, of Vernon, California, in cooperation with the CPSC, initiated a recall on September 25, 2008 of about 3,000 toy police cars, as the red paint on the toy cars contains excess levels of lead, violating the federal lead paint standard. No injuries have been reported. This recall involves toy police cars with a friction function. When the toy car is pushed forward, the car builds momentum. When released, the car moves forward on its own. The cars were sold in red/white and purple/white color combinations and measure 8 inches by 4 inches by 4 ½ inches. Item number TF820 is printed on the product’s packaging. The recalled toy cars were made in China and sold by dollar and discount stores nationwide from May 2008 through August 2008. • Shivvers Manufacturing Lawn Mower Jacks Recalled Due to Collapse Hazard
 
Shivvers Manufacturing Inc., of Corydon, Iowa, in cooperation with the CPSC, initiated a recall on September 26, 2008 of about 450 jack lift tools and kits, as the jack lifts can have stripped threats or broken nuts, which results in the shaft slipping and the mower falling from its stand-up position. This could cause serious personal injury to the consumer or property damage. Shivvers has received five reports of jack lift failures and 37 warranty claims for replacement parts. The recalled jacks are used for lifting the lawn mower during service or maintenance. Two versions of this jack model have been manufactured. Version number one was made with a nylon nut and version number two was made with a sintered bronze nut. These recalled jacks were sold separately and on Charger, JAZee and Boss riding lawn mowers. The recalled jacks were sold by Country Clippers dealers nationwide from May 2007 through June 2008 for about $150 for the jack and about $225 for the jack kits. • Wal-Mart Recalls General Electric Toasters Due to Fire and Shock Hazard
 
Wal-Mart Stores Inc., of Bentonville, Ark., in cooperation with the CPSC, initiated a recall on September 30, 2008 of about 210,000 General Electric Toasters, as a short circuit can occur between the heating element and the bread cage, posing fire and electrical shock hazard to consumers. Wal-Mart has received 140 reports of fires or sparks coming from the toasters or the toasters tripping the circuit breaker in consumers’ homes. No injuries have been reported. The recalled toasters have a chrome steel body, a black plastic base and controls with either two or four openings in the top. The GE logo is located on the front of the toasters just above the controls. Model numbers 169115 (4 Slice) and 169116 (2 Slice) are included in this recall. The model number is printed on the bottom of each toaster. The recalled toasters were made in China and sold by Wal-Mart Stores nationwide from September 2007 through July 2008. • IKEA Recalls to Repair Chest of Drawers Due to Laceration Hazard
 
IKEA Home Furnishings, of Conshohocken, Pa., in cooperation with the CPSC, initiated a recall on September 30, 2008 of about 5,000 “KVIBY” chests, as the glass drawer knobs on the chest can break either during assembly or in use, posing a laceration hazard to consumers. IKEA has received three reports of laceration injuries requiring medical attention with 11 injuries occurring outside of the U.S. The recalled KVIBY Chest is white with 4 drawers and 6 glass knobs. The recall includes chests with article number 201-080-90 and a date stamp of 0817 (first two digits indicate the year; the second two digits indicate the week) or earlier. The article number and date stamp are located on a label attached to the underside of the chest. The recalled chests were sold by IKEA stores nationwide from August 2007 through July 2008. • Aqua Lung Scuba Regulators and Adaptors Recalled Due to Drowning Hazard
 
Aqua Lung USA, of Vista, Calif., in cooperation with the CPSC, initiated a recall on September 10, 2008, of about 6000 Titan DIN 1st Stage Scuba Regulators and Titan/Conshelf DIN Scuba Adaptors, as the over-tightening of the DIN retainer by a technician during installation can result in the retainer breaking under pressure, causing a rapid escape of air from the scuba cylinder, and the regulator detaching from the scuba cylinder. This poses a drowning hazard to divers. The firm has received one report of a DIN retainer breaking under pressure. No injuries have been reported. The recalled regulators have a brass DIN retainer manufactured prior to June 2006. The recalled Aqua Lung Titan Din 1st Stage regulators have serial numbers lower than 6062501 stamped on the side of the regulator’s body. Recalled Titan/Conshelf DIN adaptors are marked “300 BAR MAX” on the side of the part. DIN adaptors marked “300 BAR MAXI” are not included in this recall. • Protocol Remote Controlled Helicopter Toys Recalled Due to Fire and Burn Hazards
 
Ashley Collection Inc., d.b.a. Protocol, of New York, N.Y., in cooperation with the CPSC, initiated a recall on September 10, 2008, of about 78,000 “Protocol” Remote-Controlled Mini Helicopter Toys, as the rechargeable battery inside the helicopter can overheat. This can result in the helicopter’s body melting, as well as a risk of fire or burns to consumers. The firm has received nine reports of incidents of the helicopter overheating, including one minor burn to a consumer’s fingertip. This recall involves the “Protocol” remote-controlled mini helicopter toys with model number starting with 1442. The helicopter is made of foam and plastic and measures about 7 inches long. “Protocol” is printed on the tail and on the side of the helicopter. “1442-X” can be found on the packaging. The remote-control component measures 5 1/2 inches by 4 3/4 inches. The recalled toy was made in China and sold by retail stores nationwide from October 2007 through December 2007. The U.S. Consumer Product Safety Commission (CPSC) is aware that some bassinets in the Simplicity recall announced August 28, 2008 include the Graco logo and “Winnie the Pooh” motif licensed by Disney Consumer Products. Consumers are urged to check their bassinets to ensure they are not using one of the recalled Simplicity models, including those with the Graco logo or “Winnie the Pooh” motif. The Simplicity bassinets with the Graco logo were sold between 2001 and 2004. The Simplicity bassinets with the “Winnie the Pooh” motif were sold between 2002 and 2008. CPSC learned that on August 21, 2008, a 6 ½-month-old girl from Shawnee, Kansas was strangled to death when she became entrapped between the bassinet’s metal bars. This is the second strangulation death CPSC has learned of involving the co-sleeper bassinets. On September 29, 2007, a 4-month-old girl from Noel, Mo. became entrapped in the metal bars of the bassinet and died. The Simplicity 3-in-1 and 4-in-1 convertible bassinets contain metal bars that are covered by an adjustable fabric flap which is attached by velcro. The fabric is folded down when the bassinet is converted into a bed-side co-sleeping position. If the velcro is not properly re-secured when the flap is adjusted, an infant can slip through the opening and become entrapped in the metal bars and suffocate. This warning does not cover bassinets produced in recent months that have fabric permanently attached over the lower bar.

Due to the serious hazard these bassinets pose to babies, CPB urges all consumers to share this safety warning with day care centers, consignment stores, family and friends to ensure that no child is placed to sleep in a Simplicity convertible bassinet covered by this warning.

• PB Heat Gas Boilers Recalled Due to Carbon Monoxide Hazard
 
PB Heat LLC, of Bally, PA, in cooperation with the CPSC, initiated a recall on September 11, 2008, of about 1,200 gas boilers, as the pressure switch could have an internal defect that could fail to shut down the boilers under vent or air inlet blockage. This switch defect could allow the boilers to produce excessive levels of carbon monoxide, posing a risk of carbon monoxide poisoning to consumers. No injuries have been reported. This recall involves the DE and PSC II Gas Fired Cast Iron Boilers with the Endura Plastics Model ES2035 pressure switch used for home heating and domestic hot water heating. The boilers were available in either natural or LP gas. The boiler rating label is located on the top jacket panel on Series PSCII models and the left side jacket panel on the Series DE models. • Regent Sports Soccer Nets Recalled Due to Entrapment and Strangulation Hazard
 
Regent Sports Corporation, of Hauppauge, N.Y., in cooperation with the CPSC, initiated a recall on September 16, 2008, of about 190,000 MacGregor Folding Soccer Goals, Mitre Folding Soccer Goals, as the fixed knot flexible openings in the soccer goal net can pose a head and neck entrapment or strangulation hazard to young children. Regent Sports and CPSC have received two reports of head entanglement, including the strangulation death of a 20-month-old child who was found with his arm and neck tangled in the soccer goal net. Both soccer goals have a foldable white frame with a white net that is attached by Velcro strips. When upright, the MacGregor goal measures 6 feet wide by 3 feet high and the Mitre net measures 8 feet wide by 6 feet high. Each has a recalled net with squares measuring 5 inches on each side. The MacGregor soccer goal has model number 97236 printed on the assembly instructions and UPC code number 029807972365 printed on the net’s packaging. The Mitre soccer goal has model number 89186 printed on the assembly instructions and UPC code number 029807891864 printed on the net’s packaging. Nets manufactured after April 2007 with 4 inch by 4 inch square openings are not included in the recall. • Various Retailers Recall Simplicity Brand Drop Side Cribs Due to Serious Entrapment and Suffocation Hazard to Infants and Toddlers
 
The U.S. Consumer Product Safety Commission, in cooperation with the firms named below initiated a recall on September 17, 2008, of about 600,000 Simplicity Brand Drop Side Cribs. Due to sizing problems with the crib’s hardware, the drop side can come off the tracks. When the drop side detaches or partially detaches, it creates a hazardous gap, which can lead to infant entrapment and suffocation. Consumers should stop using recalled products immediately unless otherwise instructed. Simplicity Inc., of Reading, Pa. (this company is no longer in business), CPSC is aware of nine incidents in which the drop side detached on cribs that are part of this recall. There have been no injuries. This recall of Simplicity drop side cribs involves models that used a different style of hardware from those cribs recalled in September 2007. The recalled model numbers include: 8620, 8745, 8748, 8755, 8756, 8778, 8810, and 8994. The recalled drop side cribs have a date code, which can be found on a label on the headboard under the mattress support, which ends in 05DH, 05GB, 06DH, 06GB, 07DH or 07GB (examples: 1806 DH or 0507 GB). The recalled model names include: Aspen and Crib N Changer Combo, Gabrielle, Camille, Providence and Shenandoah. Only the brand name “Simplicity” can be seen on the label on the headboard. The recalled cribs were made in China and sold by Department stores, children’s stores and mass merchandisers nationwide from January 2005 through August 2008. Consumers should immediately stop using the recalled cribs and find an alternative, safe sleeping environment for their baby. Consumers should return the recalled crib to the place of purchase for a refund or store credit. SFCA Inc., of Reading, Pa. is working with retailers to carry out this recall program.

• Aqua Lung Apeks Scuba Diving Regulators Recalled Due to Drowning Hazard
 
Aqua Lung USA, of Vista, Calif., in cooperation with the CPSC, initiated a recall on September 17, 2008, of about 25,000 Apeks Second Stage Scuba Regulators, as the regulators can be missing the diaphragm cover, which can cause the diaphragm to become displaced during a dive, allowing water to enter the scuba regulator. This poses a drowning hazard to divers. No injuries have been reported. This recall involves Apeks TX, ATX, and XTX second stage regulators that have never been serviced. Apeks and TX, ATX, or XTX is printed on the regulator. The recalled product was sold by authorized Apeks dealers nationwide from February 2000 through June 2008. Consumers should immediately stop diving with these regulators and visit an authorized Apeks dealer for a free inspection and free installation of a diaphragm cover, if the cover is missing. • Hanashop Counterfeit Power Cords Recalled Due to Fire and Shock Hazard
 
DCI LLC, d/b/a Hanashop, of Olive Branch, Miss., in cooperation with the CPSC, initiated a recall on September 17, 2008 of about 180 power cords as the power cord wires are significantly undersized and allow incorrect insertion of the power plug, posing a risk of fire and/or shock to consumers. No injuries have been reported. The recall involves 2 feet, three outlet power cords. The power cord is orange in color and packaged in a counterfeit yellow and green cardboard sleeve. The packaging indicates the power cord is a “12 Gauge Extra Heavy Duty” type. Model number T-4145 is printed in front and back of the packaging. The recalled power cords were made in China and sold by ebay.com nationwide from November 2006 through April 2008.Consumers should immediately stop using the recalled power cords and contact the firm for a full refund toll-free at (888) 263-2575 between 9 a.m. and 5 p.m. CT, Monday through Friday.

• Empress Arts Hooded Sweaters Recalled Due to Strangulation Hazard
 
Empress Arts Ltd., of Monterey Park, Calif., in cooperation with the CPSC, initiated a recall on September 2, 2008 of about 130 children’s hooded sweaters, as they have drawstrings through the hood in which children can get entangled, or can catch on playground equipment, fences or tree branches. In February 1996, CPSC issued guidelines to help prevent children from strangling or getting entangled on the neck and waist drawstring in upper garments, such as jackets and sweatshirts. From 1985 through June 2008, CPSC received reports of 27 deaths and 70 non-fatal incidents involving the entanglement of children’s clothing drawstrings. The cotton knit sweaters are cream colored with a blue stripe at the wrists and waist, a blue ball on the hood, and blue buttons. There is a drawstring through the hood with pom-poms at the ends. The sweaters were sold in children’s size 2T under the brand name “Empress Arts.” “Empress Baby” is sewn into the sweaters tag. The recalled garments were made in China and sold by specialty children’s stores nationwide from August 2006 through April 2008. Consumers should immediately remove the drawstrings from the sweaters to eliminate the hazard. • Rapid Reel® Portable Garden Hose Carts Recalled as the Tires Can Explode Posing an Injury Hazard
 
Eley Corporation, dba Rapid Reel of Lincoln, Neb., in cooperation with the CPSC, initiated a recall on September 02, 2008, of about 7,000 portable garden hose reel carts and wagons, as the tires can explode while being inflated, posing an injury hazard to consumers. Rapid Reel® has received five reports of tires exploding while being inflated, resulting in lacerations to fingers, arms, and legs from flying pieces of plastic. This recall involves the Rapid Reel® portable garden hose reel carts and wagons. The cart has two tires and the wagon has four. The tires have a white plastic rim and a steel ball bearing hub or a black plastic bearing hub. The recalled carts were made in China and sold by Home repair retailers and distributors such as Ace Hardware, Amazon.com, and True Value Hardware stores nationwide from April 2005 through March 2006. • Deda Elementi Bicycles with Deda Forza Bicycle Handlebar Stems Recalled Due to Crash Hazard
 
Deda Elementi S.r.l., of Italy, in cooperation with the CPSC, initiated a recall on September 03, 2008, of about 400 Deda Forza Bicycle Handlebar Stems, as the top of the bicycle handlebar stem can crack and cause the rider to lose control, posing a crash hazard. Deda Elementi has received four reports of incidents involving the handlebar stems. No injuries have been reported. The recall involves bicycles with Deda Forza black carbon fiber handlebar stems in gloss finish. “Deda Forza” is printed on the handlebar stem. Deda Forza black carbon fiber handlebar stems in matte finish are not included in this recall. The recalled bicycles and bicycle stems were sold separately by bicycle retailers nationwide from January 2008 through June 2008. Consumers should stop using the bicycles equipped with the recalled Deda Forza handlebar stems and return them to the place of purchase to receive a free replacement handlebar stem. • Circo Children’s Bobby Socks Sold Exclusively at Target Recalled Due to Choking Hazard
 
Target, of Minneapolis, Minn., in cooperation with the CPSC, initiated a recall on September 03, 2008, of about 330,000 Circo Rosette Bobbie Socks, as the ribbon on the sock can detach, posing a choking hazard to young children. Target has received four reports of the ribbon detaching, however, no injuries have been reported. The socks were sold in packs of six (pink, purple, plain white, three pairs within a box), ranging in size from 6- to 12-months, 12- to 24-months and 2T to 5T. The socks are packaged in a green and white sleeve with a Circo-brand logo on the front. The recalled socks were sold by Target stores nationwide from January 2007 to July 2008. Consumers should take these socks away from children and return them to the nearest Target store to receive a full refund.
